5月の私5：内頚静脈も拍動することがある

中心静脈をとらねばならない患者。後輩を指導して超音波下やるも拍動する逆血。自分でやっても同じ。

鎖骨下でもやはり拍動する。

もう一度超音波でやる。やはり拍動する。自信があったためカニュレーションした。CVPにつなぐとやはり静脈であった。x線でも確認可能であった。

こんなこともあるらしい。

超音波下でやるときのこつは針をみようと努力しすぎないこと。

（これにこだわるとやたら時間がかかる。）

針を動かすことで組織が動くのでそれによって自分の針先を確認すること。

（非常に大事です。）

はじめに深さを確認してそれ以上深く針をいれすぎないこと。

（大体闇雲にいれすぎると動脈を刺す人が多いです。）

I guess the best way is to check the gas before dilating...
But if you're confident, sure go a head and finish it up and you'll know with the CVP monitor..

I try to remember this scene. Actually I did trasduction using small catheter before the canulation as we ususually do if there is any doubt...

In this pt I was not able to visualize wire well by US. Usually I can and I will feel confident there.
